Paul Gascoigne's agent believes that the England legend's life is in danger as he has started drinking again.
The former Newcastle United, Tottenham Hotspur and Lazio midfielder has battled alcoholism and mental health issues throughout his life.
The 45-year-old did appear at a charity event on Thursday, but his agent Terry Baker is in no doubt of the seriousness of the situation.
"He won't thank me for saying this but he needs to immediately get help," Baker told BBC Radio Five. "Whatever's happened to him in the last five or six weeks since before Christmas.
"He's been absolutely fine, and now he isn't. I think he knows that.
"His life is always in danger because he is an alcoholic. Maybe no-one can save him. I don't know - I really don't know."
Former Manchester United goalkeeper Peter Schmeichel has called on the PFA to support the former star.
After seeing a video of Gascoigne's charity appearance, he tweeted: "This is not fun watching. Gazza needs help. Come on PFA & [PFA Chief Executive] Gordon Taylor, time to step up."
Gascoigne made 57 appearances for England during his time as a player, scoring 10 goals.
