AC Milan this afternoon confirmed that they have reached an agreement with Sevilla to sign Carlos Bacca.
Serie A side Milan are believed to have met the £21m buy-out clause in the Colombian's contract and also agreed personal terms with the 28-year-old.
Representatives from AC are now thought to be joining Bacca on his holiday in Colombia to conduct a medical before the switch officially goes through.
In a tweet from their official club account, Milan said: "Agreement reached with @SevillaFC_ENG for the transfer of @carlos7bacca, depending on Bacca passing the medical tests."
Bacca joined Sevilla from Club Brugge two years ago and has gone on to score 49 goals in all competitions for the La Liga side.
