The Senegalese international latched onto a long cross-field ball before cutting the ball back to the striker, who dispatched unmarked from around 10 yards out.
United got back on level terms when Van Persie turned Wayne Rooney's floated free kick into the net with his shoulder.
The Dutchman jumped too early and could not get his head onto the ball cleanly.
