After chances for both sides in a lively opening period, it was the hosts who were ahead at half time when Marquinho lashed the ball into the top corner from inside the area.
It took Napoli just three minutes into the second half to draw themselves level as Juan Zuniga's low effort found the net after Roma had failed to clear their lines from a corner.
The visitors took the lead with 20 minutes to play thanks to Edinson Cavani's sweet strike from distance.
