Everton defender Phil Jagielka dominated his side's end-of-season awards by winning three on Wednesday night.
The England international picked up Player of the Season, Players' Player of the Season and Goal of the Season.
Jagielka is not a regular name on the scoresheet, but his long-range strike against Merseyside rivals Liverpool in September was rewarded.
John Stones also received an accolade as he was named the club's Young Player of the Year after cementing himself into manager Roberto Martinez's first team.
Brendan Galloway, who made his Premier League debut in last weekend's 2-1 win over West Ham United, picked up Under-21 Player of the Year.
